State-Specific Steps

    Book an Appointment With Leafwell

    Speak to a Leafwell healthcare provider to get approved for an Illinois medical card online.

    The consultation costs $99, and the certification is valid for three years. You’ll get your money back if you’re not approved.

    Get Approved

    Once approved, your Leafwell healthcare provider will submit your information to the state.

    We’ll also send instructions for completing your medical card state application.

    But sit tight: We recommend waiting 24 to 48 hours after your appointment before starting your state application.

    Complete an Application With the State of Illinois

    Register on the ICTS, complete your application, and pay the applicable fees to the state of Illinois.

    Application fees for IL medical cards are currently:

    • $50 for one year
    • $100 for two years
    • $125 for three years

    State regulations require you to submit an application within 90 days of your Leafwell approval. Failure to meet this deadline necessitates an additional consultation and new certification.

    Receive Your Provisional Letter

    A Provisional Letter will be posted in your ICTS account within 24 to 48 hours.

    This letter will allow you to purchase medicine at licensed dispensaries.

    The state of Illinois can take up to 90 days to fully approve your application. When that happens, you’ll receive an official digital medical card. Physical cards are no longer issued.
